A personalized assessment just for you

The assessment gives you a chance to talk to a doctor or nurse practitioner about your health. It also provides us with a snapshot of your health that we can share with the doctor. By having this information, the doctor can provide care that’s right for you. Other benefits include:

  • One-on-one time with a doctor or nurse practitioner to review your health history and answer your questions
  • A thorough health exam
  • Tools to help keep your health on track

It’s important to note that this assessment doesn’t replace the preventive care visits that you schedule with your primary care physician.

Reap the benefits of a health assessment that’s just for you

By scheduling an assessment, you can:

  • Find out if you have any diseases or conditions
  • Learn ways to manage your health and diseases
  • Get help coordinating your health care services
  • Increase your options for getting health care
Know what to expect at your visit

It’s always helpful to know in advance what to expect. Your assessment will only last approximately 45 minutes. Should you choose, you can have a friend or family member with you during the entire visit. We want you to feel comfortable. And to make you feel more comfortable, you don’t have to wear a hospital gown and you can wear your regular clothes.

During the visit, a doctor or nurse practitioner will:

  • Check your height and weight, and take your temperature,
    blood pressure and pulse
  • Ask you questions about things like what you eat, how well
you sleep and whether you exercise regularly
  • Give you a written summary of your visit and details on our
programs and tools to help you stay healthy

The good news – the doctor or nurse practitioner will not draw your blood.
